DUTIES & RESPONSIBILITIES: Interprets and reviews for any appropriateness before charging, preparing and dispensing any medications to patients.
Ensures all computer data entries are done correctly and report to Pharmacy Manager if there are any discrepancies.
Counsels patients in their medication therapy (including bedside dispensing).
Ensures cleanliness of the pharmacy and work counters.
Prepares and supplies all medications to outpatients, inpatients, wards and other departments as required.
Ensures sufficient stocks are available in pharmacy and inform the person-in-charge to purchase/order drugs supply and other supplies (e.g. consumables, stationeries etc.) and to ensure that no disruption of supply takes place due to shortage.
Checks the labels of all containers in order to ensure that labels are clear and contain all the necessary information.
To be responsible for medications under Patient Assistance Program, as assigned by their superior.
Informs the Pharmacy Manager of any new drug request or of any abnormality in prescriptions encountered.
Ensures that no substitution of any single prescribed drug is made without the approval of the Consultant and Pharmacy Manager.
Checks that all Psychotropic Substances (PPS) are kept under lock and key in accordance with the regulations and that all the books for these purposes are entered up to date and kept in the proper place for case of reference and checking under the supervision of the Pharmacy Manager.
Monitors patients owing medication list and ensure that owed medication reach patients safely and in a timely manner.
Provides ward Pharmacy services if assigned.
Performs medication preparation unit of dose, medication order discontinuation in the hospital information system and medication reorder.
Audit administration of medication by nurses against the medication preparation administration record (MPAR).
Process discharges at the ward level and carry out bedside dispensing.
Raise charges for new medication order.
Monitors the usage of PPS and ward stock used and ensure charges are entered accordingly on daily basis.
Collects and compiles data/statistics required by Pharmacy Manager for departmental reports when assigned.
Performs physical stock count whenever assigned by Pharmacy Manager.
Checks the accuracy of physical and electronic stock balance or transfer and report to Pharmacy manager for any discrepancy
Prepares and reconstitutes cytotoxic drug when assigned by Pharmacy Manager.
Ensures that all pharmacy stocks are not expired.
Ensures stocks are arranged properly and systematically in appropriate containers and that FEFO is practiced at all times.
Assists pharmacy manager in stock rotation within and outside the hospital.
Assists the Pharmacy Manager in maintaining the standard of pharmacy practices in accordance with professional practice requirements, legislations and hospital policies & standard operating procedures.
Performs other duties as requested by the Pharmacy Manager and the Management.
