Job Description

Job purpose

For nurses who are motivated to expand their knowledge and career development, we offer opportunities to enhance your skills and transition into the role of a palliative care practitioner.
  • Duties and responsibilities
  • A. Clinical

  • Provide compassionate and professional nursing care to ASSISS patients in the community setting and during home visits, abiding to the institutional criteria and policies.

  • Educate patients and their families on necessary nursing skills for home care.

  • Liaise with ASSISS clinical teams on patients medical conditions so that proper care can be administered, abiding to the institutional criteria and policies.

  • Discuss with the senior practitioners and medical officer with regards to patient care needs and should there be any arising problems.

  • Maintain accurate patient records and documentation according to the institution format.

  • Assist in administrative work such as stock keeping, ordering of essential items and medications, and manage medical equipment.

  • Practice timely response and action on matters needing attention in the unit and components of ASSISS.
  • B. Professional / Procedural Aspect

  • Perform nursing care procedures as required and ensure skills are up to date.

  • Initiate first contact via telephone with the patient or family member within 24 hours of referral followed by arrangement of the first assessment visit or follow up.

  • Update patients records after every home visit and telephone contact with the patient or family member with details of care given, patients condition and other relevant matters.

  • Keep track of surgical stock and medication stock.

  • Assist in loaning out, up keeping and receiving medical equipment.

  • Keep up to date with new clinical developments in nursing care through reading of current materials and participation in educational programs and courses.
  • Qualifications
  • Diploma in Nursing registered with Malaysian Nursing Board
Practicing / Annual Practicing Certificate (APC)
  • Area of specialty Nursing and community nursing
  • Skills and Knowledge
  • Able to work independently in care management of patients
Able to communicate effectively within the team
  • Strong interpersonal skills and a people-oriented person
Able to drive independently (preferably own dependable vehicle)
  • Working conditions

  • The position requires the holder of the job title to work in a community setting, whereby he/she has to visit patients in their homes, located within the coverage of ASSISS.

  • Attend all ASSISS functions.
  • Physical requirements

  • Physically healthy.

  • Emotionally and psychologically stable and strong.
  • WORKING CONDITIONS

Candidate is expected to work from:


Monday to Friday : 8.00am - 5.00pm

Saturday : 8.00am 1.00pm

Job Type: Full-time

Pay: RM2,500.00 - RM3,000.00 per month
